Register With The Practice?
New patients can now pre-register with the practice online. You MUST bring into the practice some form of proof of identity (original) and domicile before we are able to accept you as a
regular NHS Patient.
If you wish to register with the Practice, please visit us and talk to our receptionists. It would be a great help if you could bring with you your up-to-date medical card. If you cannot find it, do not
despair. Simply see our receptionist and ensure you can supply the following details:
- Full name, including previous names
- Date and place of birth
- Last address
- New address and postcode
- Last name and address of your previous doctor
- NHS number if known - not the same as your National Insurance number
- Identification document (original)
NHS Counter Fraud and Security Management Services
We are working with the NHS Counter Fraud and Security Management Services to reduce the number of people who claim NHS care fraudulently. We now require you to provide proof of identity and
entitlement to NHS care. A copy of your proof of identity and entitlement to NHS care will be stored electronically in your medical records on our computer system.
YOU MUST PROVIDE YOUR PROOF WITHIN 5 WORKING DAYS following the completion of your registration application. Failure to provide the documentation will result in your application being rejected.
The following proofs of identity will be accepted (in preferred order)
- European Health Insurance Card (EHIC)
- Passport (including visa for stay in the UK)
- NHS Medical Card
- Photo Driving Licence
- Birth Certificate
- Marriage Certificate
- National Insurance ID Card
- Evidence of benefit entitlement
- Wage slip with employer details
